Aim: The heterogeneity of the Amyotrophic Lateral Sclerosis (ALS) clinical phenotypes leads to difficulties in early diagnosis. ‘split hand’ sign is defined as thenar muscles that are more prominently affected by hypothenar.  Material and Method: Seventy-five who diagnosed ALS our clinic included study. ENMG 70 healthy controls: Distal motor latency (DML), compound muscle action potential (CMAP) amplitude, velocity median, ulnar, peroneal tibial nerves; distal latency, sensorial evaluated. Ulnar/median DML ulnar/median CMAP amplitude ratios examined.
 Results: group, DMLs peroneal, nerves significantly longer, amplitudes smaller than controls. sensory conductions sural not statistically different between groups. ratio was lower (0.73/0.80;p=0.003); while greater (1.40/1.11; p=0.002).
 Conclusion: Prolonged reduced observed period. present also support presence split-hand phenomenon even period limb-onset (both upper lower). These suggest studies electrophysiologically detected important clues for diagnosis case heterogeneous phenotype.
